Enabling Healthy Change
Joined up care at the right time

Our purpose

Is to enable healthy change in the people and places it is needed most. We achieve this through delivering education, skills training and community-based behavioural activation support services. We put people at the centre of our thinking and understand the wider determinants of their health. With our support, people and professionals are more able to work together to achieve sustainable change and reduce health inequalities.

Our philosophy

Is that our culture and our leadership behaviours mirror the way in which we support the people we work with. We build strong relationships with our partners and embed within the neighbourhoods and pathways we support. We offer a holistic approach to health and wellbeing, focusing on what matters to people: a quick response, meeting at a time and place of their choosing and outcomes measured with an evidence-based tool.

Our Values

Have remained the same since we began in 2008. We share a passion to enable healthy change, which means that you will see and experience behaviours that demonstrate our values lived out every day.

Our organisation

Consists of a network of autonomous teams, our home teams. Working within the boundaries set by our operating framework (‘what’ we do) our teams decide ‘how’ they deliver to our quality standards. Working in this way requires every team member to get more involved in the decisions being made.

South Tyneside Social Prescribing Service

Social prescribing is an approach that connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.

Our teams give people time, focusing on ‘what matters to me?’ to coproduce a simple personalised care and support plan, and support people to take control of their health and wellbeing.

Over the last 5 years we have worked with over 15,000 people. 78% of the people we work with live in the most deprived wards in England (IMD 1-2), 92% live in deprived wards (IMD 1-4).
