Many things affect our physical, mental, and social health. Some of the things that people often like to talk about with us are:

  • Feeling down, stressed or anxious
  • Managing health conditions and symptoms
  • Keeping active
  • Benefits, money and housing problems
  • Living with chronic pain
  • Making healthier choices
  • Reducing smoking or alcohol
  • Loneliness
  • Caring for someone
Social Prescribing Service

Our Social Prescribing teams take time to listen to you which helps us to understand your situation, and what matters to you. This means we can:

  • Identify what is affecting your well-being and how you would like things to be different.
  • Build your knowledge, motivation and confidence to make changes.
  • Set and achieve personal goals to improve your well-being.
  • Connect you to activities, groups and opportunities in your area.
  • Develop ways you can manage your mental and/or physical health conditions.
  • Support you to navigate your care, if multiple services are involved.

Everything discussed will be kept confidential unless you discloses something which highlights that someone is at risk, we may need to share this with relevant organisations.

Anyone aged 18 or over (including transition years) living in South Tyneside, or registered with a South Tyneside GP, can access our services free-of-charge.
